Before you create the labels, choose the option change fields, then click create labels it will open a document and ask you to make any changes you want, open the document and from the top choose the fields you want to add (in your case you would choose for mother maiden name = FNameW, and then the phone types you want) when your done go back to the program and click OK, and your document should get created.
