How do I get my schools facility code for CIR?

As of version (09/10/23)  the program asks for a school facility code when using CIR.

Go to the following website https://immunize.nyc/provider-client/servlet/PC?PCFunction=DosesAdminReport

Click on the "MM/Covid" tab and on the bottom it says 

"If your facility (SchoolName, **facility code**) is planning to participate in the VFC program, your practice needs to enroll in the VFC Program"

after you sign in you will find your facility code on the upper left.
